Premature Ovarian Insufficiency (POI) – 2024 Update
Premature Ovarian Insufficiency (POI) is not menopause or ovarian failure. It reflects fluctuating ovarian function, with rare spontaneous pregnancies possible.
Key Points:
Diagnosis: FSH ≥25 IU/L + ≥4 months amenorrhea; single test often sufficient
Etiology: Always check genetic, autoimmune, or iatrogenic causes (Karyotype, FMR1, 21-hydroxylase antibodies)
Hormone Therapy: Life-saving; higher-dose estrogen (transdermal preferred); add progesterone if uterus present
Fertility: Egg/embryo donation is gold standard
Local Care: Estriol cream for dryness, dyspareunia, urinary symptoms
Monitoring: Bone, heart, mental health; therapy adherence essential
